Is there a stain coat under the poly?...sounds like the prep before the poly was applied was either not done properly, or the stain coat was not dry enough....the choice of Minwax Poly isn't a terrible one...it's used by alot of floor guys...a decent enough product..but only when the directions are followed ...meaning adequate dry times of all coats....it can take several days to a week to fully cure...depending on temp/humidity, etc., I've seen it fail before when the finisher is trying to stain a hard wood very dark with oil stains..or a "home brewed" stain.....lack of experience is the cause...not the Minwax Poly, which is a modified Alkyd varnish product. If there is no stain...you might be able to sand it down and feather the worst areas, and simply recoat.......but it sounds like the adhesion of the finish failed for some other reason.....
