Your approach will work, but I suggest that you veneer both sides of the plywood to keep even stress and reduce the chance of warping, also if the table has a skirt then skirt the leaves. I also like to edge band the plywood with at least 1/2 thick hardwood to conceal the edges and provide a good base for the leaf pins and holes.
