Sure it can be sand blasted, and it will be a nice and smooth as old barn board, and the lead paint dust will create an invromental hazard. Not to mention that sand will be falling onto the kitchen tables for the next fifty years. You are stuck with paint remover or new trim. Have you considered a heat gun, watch out for lead paint fumes from the heat though. Wear a good mask and use lots of ventelation too.
