The part that seems to be a washer might actualy be the threaded insert which is supposed to secure the mirror in the selected position of the user. Try to loosen the adjuster bolt and pull out at the same time. It may be that the threads are stripped. Try both adjusters possibly only one was stripped. I know of no lock washers that were used in this application. If you pry gently with a slender tool, (butter knife) as you reverse the bolt you may notice that it will start to back out.
