I understand what you are asking, it is tough to tell you what to do without seeing the piece and perhaps working on it to develope an idea of exactly what you have in mind. If you have not already done so I would first recommend a good cleaning with soap and water followed with a good rinse,then if it is milk paint under a later coat of some other type you can strip the top coat an preserve the milk paint or you can continue to sand carefully exposing more of the milk paint without going to bare wood. Once you have it looking like you want it should be clear coated with blonde shellac which will add some gloss and protect the bare areas.
